Why Integrated Care Transforms Outcomes
Traditional healthcare fragments your treatment across disconnected providers who rarely communicate. Our collaborative care model places your therapist, psychiatric provider, care coordinator, and social worker on a single clinical team with shared records, daily consultation, and unified treatment planning. Research consistently demonstrates that integrated behavioral health care reduces emergency department utilization, improves chronic disease management, and achieves better mental health outcomes than siloed treatment models.
Unified Clinical Team
Your therapist, psychiatrist, care coordinator, and social worker operate as a single team within our shared electronic health record. Daily clinical huddles ensure every provider is aligned on your treatment trajectory, medication changes, and recovery milestones. No more repeating your story to disconnected specialists.
Warm Handoff Protocol
When your therapist identifies a need for psychiatric evaluation, medication adjustment, or social services support, the referral happens in real time through a warm handoff — not a cold referral letter. You meet the next provider during the same visit or within 48 hours, ensuring continuity and eliminating the care gaps where patients are most vulnerable.
Measurement-Based Care
Every session incorporates validated clinical outcome measures (PHQ-9, GAD-7, PCL-5) tracked longitudinally so your treatment team can objectively monitor progress, adjust interventions proactively, and demonstrate treatment effectiveness. Evidence shows measurement-based care doubles response rates compared to treatment as usual.
Comprehensive Behavioral Health Under One Roof
From individual counseling and psychiatric medication management to substance use recovery and social services navigation, every service at Health Alliance is designed to work together for whole-person healing.
Behavioral Health Counseling
Individual, couples, and family therapy using evidence-based modalities including Cognitive Behavioral Therapy, Dialectical Behavior Therapy, EMDR for trauma, and Motivational Interviewing. Our licensed therapists specialize in depression, anxiety, PTSD, co-occurring disorders, and relationship dynamics within the collaborative care framework.
Psychiatric Medication Management
Board-eligible psychiatric prescribers conduct comprehensive diagnostic evaluations and provide ongoing medication oversight. Every prescription decision is made in consultation with your therapist and primary care physician, ensuring pharmacological treatment supports your broader therapeutic goals without conflicting with other medical regimens.
Substance Use Recovery Support
Outpatient substance use disorder counseling seamlessly integrated with mental health treatment. Our program includes Medication-Assisted Treatment coordination, peer recovery specialist support, group and individual counseling, relapse prevention planning, and connection to community recovery resources throughout Hennepin County.
Primary Care Navigation & Social Services
Dedicated care coordinators and social workers help patients navigate complex medical systems, manage chronic conditions alongside mental health treatment, access insurance benefits, secure housing and food assistance, arrange transportation, and connect with community resources. We address the social determinants of health that profoundly impact treatment outcomes.
Measurable Impact Since 2016
Our integrated model delivers outcomes that fragmented care systems consistently fail to achieve.
Stories of Whole-Person Healing
“For years I bounced between a therapist, a psychiatrist, and my primary care doctor who never talked to each other. My anxiety medication conflicted with my blood pressure treatment, and nobody caught it. At Health Alliance, my whole team meets weekly to discuss my care plan. Within three months, my anxiety scores dropped by half and my blood pressure stabilized. I finally feel like someone is looking at the complete picture.”
— Maria T., Patient since 2022“When I came to Health Alliance I was dealing with depression, alcohol dependence, and had just lost my housing. My therapist introduced me to a social worker that same day through a warm handoff — not a referral letter that would take weeks. Within a month I had stable housing, was in outpatient recovery, and started individual therapy. That level of coordination saved my life.”
— James R., Patient since 2023“My teenage daughter was struggling with self-harm, school refusal, and family conflict. The child behavioral health team at Health Alliance worked with her individually, brought our family into therapy together, and coordinated with her school counselor. They treated the whole system, not just her symptoms. She is thriving now and we are a stronger family because of it.”
