Confidential Mental Health & Wellness Screening

This brief, evidence-informed screening tool helps identify areas where professional behavioral health support may improve your quality of life. Developed in alignment with validated screening instruments including the PHQ-2 for depression, GAD-2 for anxiety, and additional domains assessing sleep quality and social connectedness. Your responses are processed entirely in your browser and are never stored, transmitted, or associated with any identifying information.

Before You Begin

This screening is designed for adults aged 18 and older and takes approximately two to three minutes to complete. It is not a diagnostic instrument and does not replace a professional clinical evaluation. The results provide general guidance about whether integrated behavioral health services at Health Alliance may be appropriate for your situation.

If you are in immediate crisis or danger: Please call 988 (Suicide & Crisis Lifeline), text HOME to 741741 (Crisis Text Line), or go to your nearest emergency department. You can also reach our crisis support line at (612) 555-0287 during office hours.

Confidential Wellness Screening

This brief, evidence-informed screening helps identify areas where integrated behavioral health support may be beneficial. Your responses are completely private and are not stored or transmitted. This is not a clinical diagnosis.

Understanding Your Screening Results

The screening evaluates six domains that clinical research has identified as key indicators of overall mental and emotional wellbeing. Each domain is scored on a four-point scale reflecting the frequency and severity of symptoms over the preceding two-week period. The composite score provides a general indication of whether professional support may be beneficial, with higher scores suggesting greater potential benefit from integrated care services.

Low Indication (Score 0–4)

Your responses suggest generally positive mental and emotional functioning. Maintaining healthy lifestyle habits, social connections, stress management practices, and regular preventive wellness activities supports long-term resilience. Preventive consultations and stress management workshops are available for individuals seeking proactive mental wellness maintenance.

Moderate Indication (Score 5–10)

Your responses indicate that you may be experiencing moderate levels of emotional distress that could benefit from professional support. Integrated behavioral health counseling can help you develop evidence-based coping strategies, identify underlying patterns, and address concerns before they escalate. Individual therapy, care coordination, and peer support groups are recommended starting points.

Elevated Indication (Score 11–18)

Your responses suggest a higher level of emotional distress warranting prompt professional evaluation. Our integrated care team can provide a comprehensive biopsychosocial assessment, develop a coordinated treatment plan that may include therapy, psychiatric medication management, and social services navigation, and connect you with crisis support resources if needed. We encourage you to begin the intake process or contact our office directly.

The Science Behind Integrated Screening

This screening draws from established psychiatric assessment instruments that have been validated across diverse populations in primary care and behavioral health settings. The Patient Health Questionnaire (PHQ) and Generalized Anxiety Disorder assessment (GAD) are among the most widely used screening tools in clinical practice, endorsed by the American Psychiatric Association and the United States Preventive Services Task Force for routine screening in primary care.

We supplement these standard mood and anxiety domains with sleep quality and social connectedness assessments because research consistently demonstrates that these factors are powerful modulators of mental health outcomes. Chronic sleep disruption is both a symptom of and a risk factor for depression, anxiety, and substance use disorders. Similarly, social isolation and weak support networks are among the strongest predictors of poor treatment response and relapse across virtually all behavioral health conditions.

At Health Alliance Integrated Care, we use these same validated instruments as part of our measurement-based care approach, administering them at every clinical session to track treatment response objectively and adjust interventions in real time. This data-driven methodology has been shown to double treatment response rates compared to care guided by clinical judgment alone.
